Getting ready for your visit.

We are ready to provide you with our best care whether we’ll be seeing you in our Bellevue, Kirkland or Issaquah offices.

Please remember to bring your photo identification, a list of your current medications and your insurance card to your appointment.

Before your visit, please click on the links below for our patient registration forms. Print out and complete these forms and then bring the completed forms to your first appointment. Bringing the completed forms with you to your appointment will decrease the time you need to spend in our waiting room before your appointment.

If you do not complete these forms before your appointment, please arrive an extra fifteen minutes prior to your appointment to complete your paperwork.
